In our real-life version of snakes and ladders we had slid down another snake and were back to the start. I was 35 when our daughter was born and we took for granted that we would have a second child. I got pregnant almost as soon as we started trying but I had no pregnancy symptoms at all and a gut feeling that something was wrong.

The sonographer started the scan and we waited anxiously for the image to come into view. But there was nothing there, just a black screen. We were at a private clinic and were offered neither information nor support. Instead, we were taken to a side room with a box of tissues and handed an envelope containing the bill for the scan. I wanted it to be over with as soon as possible so we could start trying again so I had surgical management, which at least meant there was minimal pain or bleeding.

We knew couples who had experienced a miscarriage and had gone on to have a baby soon after so we moved on relatively quickly. We thought we had had our bit of bad luck and next time all would go well. A few months later I was pregnant again.

As the frequency of multiple pregnancies is increasing, every obstetrician has to know that the correct, accurate, and timely determination of gestational age, chorionicity, and amnionicity has significant importance in the management of a multiple pregnancy. Surveillance, complications, outcome, morbidity, and mortality are totally different in a monochorionic and a dichorionic pregnancy. In this chapter, we will present the sonographic figures that are visualized in the first trimester in a multiple pregnancy and help us define the gestational age, chorionicity, and amnionicity.

We will classify them into two periods: the early first trimester, including the 10 first weeks of gestation and the late first trimester including the period between the 10th and 14th week of gestation. Finally, we will review some interesting, although infrequent, cases from the literature, showing that pitfalls in the determination of both chorionicity and amnionicity exist and highlighting the importance of being aware of their subsistence.

A dating scan is an ultrasound examination which is performed in order to establish the gestational age of the pregnancy. Most dating scans are done with a trans-abdominal transducer and a fullish bladder. If the pregnancy is very early the gestation sac and fetus will not be big enough to see, so the transvaginal approach will give better pictures. Dating scans are usually recommended if there is doubt about the validity of the last menstrual period.

By 6 to 7 weeks gestation the fetus is clearly seen on trans-vaginal ultrasound and the heart beat can be seen at this early stage 90 to beats per minute under 6 to 7 weeks, then to beats per minute as the baby matures. Ultrasounds performed during the first 12 weeks of pregnancy are generally within 3 – 5 days of accuracy. The most accurate time is between 8 and 11 weeks gestation.

This is because the fetus is growing so quickly that there is a big difference in size from week to week. However, the accuracy of the ultrasound examination is always dependent on the skill of the sonographer and the quality of the equipment. The EDD from the early dating scan is used – if the last menstrual period is not known or is unreliable, or the dating scan differs from the last menstrual period dating by more than 5 days.

Ultrasound examinations from 12 to 22 weeks are regarded as being within 10 days of accuracy or up to 10 days earlier or 10 days later than the woman’s calculated due date.

All stages of pregnancy have their own signs of indicating a healthy pregnancy. These signs assure a pregnant woman that the foetus is developing well and the pregnancy is progressing as anticipated. One of the first such indicators that a mother-to-be looks forward to is a yolk sac during her first ultrasound scan. The yolk sac is a foremost structure that is visible inside the gestational sac which envelopes the emerging foetus and the amniotic fluid. The gestational sac resembles a white circumference around an indistinct centre.

The yolk sac performs the role of providing the preliminary nourishment to the growing foetus. It is also responsible for early circulation and delivers essential nutrients to the embryo through a method known as vitelline circulation. The foetal circulatory system develops at around 12 weeks of gestation , and the yolk sac ceases its circulatory function around this time.

The yolk sac is not discernible until about five to six weeks of gestation. Sometimes the yolk sac is not visible during the first ultrasound scan. If no yolk sac is seen in the ultrasound , it could merely mean that the gestational age has been misread. This may be the result of an error in recalling the date of the last menstrual period or if a woman has irregular periods. If your doctor suspects that there is no visible yolk sac when you are presumed to be at six weeks of your pregnancy, she may attribute it to inaccurate gestational age, and you will be suggested to come back for a scan in two weeks.

If the pregnancy is progressing well, a yolk sac and possibly a foetal pole should be visible by then.
